Grantee Spotlight: Project H.O.P.E.
For many patients with a chronic disease, learning about proper nutrition and how to cook nutritious meals is a key to maintaining one’s health. The CVS Caremark Charitable Trust and the National Association of Community Health Centers (NACHC) are proud to provide an “Innovations in Community Health” grant to Project H.O.P.E in Camden, NJ, for their intensive Chronic Disease Management program. Through the grant, Project H.O.P.E provides nutrition education and cooking classes to individuals who are homeless or at-risk of being homeless, in collaboration with Cathedral Kitchen, the largest soup kitchen in Southern New Jersey.
